Instructions for Completing the Template

  • Complete the State governing law for interpretation and enforcement.

  • Complete Appendix A:

    • List each Trademark Name, Country/Location, Registration Number, Registration Date, Application Number, and Application Date.

  • Arrange for a notary public to notarize the Assignor’s signature.
