  • Insert your LLC’s full legal name, ensuring it complies with Florida naming rules (e.g., must include "LLC" or "L.L.C.").

  • Fill in your principal place of business address, which can be within or outside Florida.

  • Provide a mailing address for official communications (can match principal address or be different).

  • Designate a Registered Agent located in Florida and obtain their written consent to serve.

  • List all initial members of the LLC with their full legal names.
